Teleservice & Virtual Service Site Request & Agreement Form
VISTA members are allowed to serve from a virtual site but must be located within the commuting area of the geographic community where the sponsor or site/assignment provides service and are physically present to meet with their supervisor and/or attend community events.
In order to perform teleservice or virtual service work, VISTA members must complete a Teleservice and Virtual Service Site Request online at the My AmeriCorps Member Portal. The completed form is then sent to site supervisors and project directors through the eGrants Portal.
This form serves as a checklist to assess the safety of a teleservice or virtual service site arrangement and serves as the agreement form to ensure compliance with the VISTA Teleservice and Virtual Service Site Policy. If you would like to preview the information needed to successfully complete the form online, you can view the fields here.
